Adoration Home Health Care Team & Services
Our interdisciplinary Care Team is comprised of an expert panel of Clinical Managers, Registered and Licensed Practical Nurses, Medical Social Workers, and Occupational, Physical, and Speech Therapists from your community. In addition, our agencies work with a Medical Director who assists in evaluating the care we provide and offers recommendations for improvement. Our team empowers individuals to regain their independence in the comfort and safety of home. In a study concluded by CareJourney, home health care is linked to better long-term outcomes related to cost, as well as lower readmission rates, emergency department rates, and mortality.
Our team works closely with your physician(s) to create a care plan that addresses your clinical and recovery requirements and adheres to your wants and needs. In addition, our team reviews and revises your plan of care on an ongoing basis so that it always addresses your concerns. You will always have a copy of your current plan of care while you are with us
Core Services We Provide:
- Specialized Nursing Care. Our RNs and LPNs provide medication management, wound care, chronic disease education, nutritional/pantry assessment
- Physical Therapy. Our PTs provide fall prevention/trip hazards, mobility restoration, post-surgical recovery.
- Speech Therapy. Our SLPs treat swallowing disorders, and speech recovery after stroke.
- Occupational Therapy. Our OTs help patients with activities of daily living (ADLs), adaptive equipment training.
- Social Services. Our SWs advocate for patients by providing emotional support, community resources, and ensuring they receive the proper support to maintain independence at home.
