Continue CareRx is for patients who are taking 5 or more medications and need a solution for taking their prescriptions on the right day, at the right time.
How long until I get a call back?
Enrollment inquiries will receive a response on the same business day during our normal business hours of 8:30am-5pm, Monday-Friday EST.
What information do I need to sign up?
- Medication list
- List of providers with their contact information who prescribed your medication
- Pharmacy insurance information
- Previous pharmacy information
Can I enroll on behalf of a family member?
